Blu-ray menus ok no video plays but same project to DVD is fine

I have been using Encore CS6 for a couple years doing the same steps to make DVDs and Blu-rays.

Encode as media Blu-ray MP2 from Premiere Pro CC

Create project in Encore and import mv2 and wav files. 

Add menus etc. check project and test in Preview.

Set build format to Blu-ray and output to Image.

Create blu-ray ISO file.  Burn to Blu-ray disc

Change build format to DVD, output as image.

Create DVD ISO file. Burn to DVD.

All just peachy. This ONE video is killing me.  All works as expected in Encore Preview. DVD works fine.

Blu-ray displays all menus fine but when you select anything to play no video plays and you are taken back to the main menu, ???????????????

I have tried recreating the Blu-ray ISO several times and burning on several discs. Same result. Other projects work just fine all using same settings.

Any ideas????
